Applications for operators’ certificates of public convenience and necessity shall be made to the Secretary of the Board on blanks provided by said Secretary and shall set forth the name and address of said applicant, the trade name, the name of every person, firm, or corporation interested in said business, the class of vehicle for which an operator’s certificate is desired, a full and complete description of all equipment to be used, and any other information required by the Secretary.
