711.02 PERMIT APPLICATION.

   The application for the permit shall include:

   (a)   The name, address and telephone number of the owner.

   (b)   The name, address and telephone number of the driller.

   (c)   A performance bond in the amount of five thousand dollars ($5,000) made payable to the City of Conneaut.

   (d)   Presentation of a City Zoning Permit and such State of Ohio permits as required.
