§ 118.155 REPORTING REQUIREMENTS.
   (A)   The grantee shall, at the time payment of franchise fees are made, file with the City Clerk a report on the revenue upon which the franchise fees are based.
   (B)   The grantee shall, at least semi-annually, plus any other time upon request, submit to the City Clerk the record of all complaints received by the grantee that it is required to maintain by § 118.114 and the record of all service interruptions that it is required to maintain by § 118.112. Upon request, the grantee will provide the city government with additional information on any complaint or service interruption and the corrective action taken in response thereto.
   (C)   The grantee shall file with the City Clerk copies of any reports or other documents, pertaining to cable service rates and charges for subscribers in the city, prepared by the grantee for submission to the FCC, at the time they are submitted to the FCC. The grantee shall, upon request, provide the city government with copies of any other reports or other documents it has submitted to or formal communications it has received from the FCC or any other state or federal agency except the Internal Revenue system, pertaining to its cable system in the city.
   (D)   The grantee shall, upon request, provide the city government with a copies of true and accurate maps or plats of the cable system and of any proposed cable system extensions or line changes.
   (E)   The grantee shall, upon request, provide the city government with any other information requested by the city government that may be useful for ensuring the grantee is in compliance with this chapter and its franchise agreement.
