A permit provided for in this subchapter shall state on its face to whom it is issued, the date it will expire, the address and location of such dance hall or place where the dance will be held, and whether the permit is issued for a dance hall or a public dance. It shall be signed and sealed by the Assessor and Collector of Taxes.
