Any person desiring a permit to operate a dance hall shall file with the Chief of Police a sworn written application for such permit, which application shall state:
(A) The location of the place, room or floor and the size thereof which is proposed to be used for such purpose.
(B) The name and address of the applicant and the length of his residence at such place.
(C) Whether or not the applicant has been convicted of a felony or misdemeanor, and if so, the nature of the offense.
(D) If the applicant is a firm, partnership or corporation, the information required in divisions (A), (B) and (C) of this section shall be given with reference of each member thereof and with reference to the operator or person in charge of the dance hall.
(E) The previous occupation of the applicant for a period of five years next preceding the filing of the application.
