The cable operator's proposal to serve the area shall include, but is not limited to:
(A) Provisions for regular reports, plans, technical tests, and performance of the operator;
(B) Insurance requirements;
(C) Rules governing the transfer of ownership;
(D) Provisions providing for facilities, equipment, and complaint procedures;
(E) Provisions for channel capacity for public, educational, and governmental use, and categories of programming offered and comparisons with rate levels of surrounding communities;
(F) Customer service and construction-related needs;
(G) Provisions to substantiate the cable operator's physical means to comply with the requirements set forth in this chapter;
(H) An estimate of annual gross receipts and proposed franchise fee; and
(I) A statement of place of payment for cable customers and the cable operator's availability through local telephone exchange.
