(A) No person shall be employed to perform emergency medical personnel service unless such person shall have been properly licensed by the Oklahoma State Health Department.
(B) On or before July 1 of each year all operators shall file a schedule containing a list of the names and addresses of all emergency personnel employed by the operator. In addition, said list will contain the Oklahoma State Health Department license designation given to the employee along with the expiration date of said license.
