§ 91.08 FORM AND CONTENTS OF CERTIFICATE.
   Every certificate issued shall be signed by the Mayor, attested by the City Clerk and shall contain the name and address of the applicant, and the number of vehicles authorized to be operated pursuant to such certificate.
