Every person to whom Section 561.01 is applicable, who comes within the City from any point outside thereof, whether in transit through the City or not, shall report to the Chief of Police of the City within one (1) week after taking up residence in the City and shall furnish to the Chief of Police a written statement signed by such person containing his true name and every other name or alias by which such person is or has been known, a full and complete physical description of himself, the name of each crime described in Section 561.01 of which he has been convicted, together with a name of the place where such crime was committed, the name under which he was convicted, and the year of the conviction therefore, the address of his residence in the City, if any, or the address or location of his intended residence, stopping place or living quarters in each thereof, the length of time he intends to remain or reside within the City, and the date of his most recent release from incarceration for the commission of a crime described in Section 561.01.
