The hauler shall maintain the following types of insurance coverage and in the following amounts:
(a) Fire and lightning insurance, with uniform extended coverage endorsement, for all equipment used in the City;
(b) Liability insurance to the extent of five hundred thousand dollars ($500,000) per occurrence for property damage, including loss of use;
(c) Insurance adequate to meet liability requirements under the Illinois Structural Work Act as required;
(d) Worker' Compensation Insurance within statutory limits prescribed by the Illinois Workers Compensation Act from time to time;
(e) Employer liability insurance at least one hundred thousand dollars ($100,000); and
(f) Comprehensive automobile liability insurance to the extent of one million dollars ($1,000,000) per occurrence against liability for bodily harm or death, and three hundred thousand dollars ($300,000) per occurrence against liability for property damage, including loss of use.
