Applications for licenses shall be made to the Mayor in writing signed by the applicant if an individual or by a duly authorized agent thereof if a club or corporation and verified by oath or affidavit and shall contain such information which will illustrate that the applicant qualifies for licenses for machines in the State of Illinois. Said application form shall be provided by the City to the applicant.
