All applications to the City Clerk for a License issued pursuant to this chapter shall be made on a form provided by the City Clerk and include the following:
(a) Name of applicant.
(b) Brief description of the business and of the goods to be sold.
(c) Name and address of employer, if any.
(d) Description of previous licenses issued by the City or other municipalities within the last three years.
(e) Copy of valid driver’s license or other photo identification for all employees.
(f) Copy of current permit issued by local health department.
(g) Certificate of insurance evidencing the applicant has liability insurance that will cover the applicant’s proposed business activity under this license.
(h) Acknowledgment that issuance of license is contingent on City’s approval after a criminal background check.
