An applicant for a permit hereunder shall file with the town an application with the following:
(A) Engineered plans locating the work area and street(s) of the project site;
(B) Specifications detailing and locating the work to be done;
(C) The name and address of the party or corporation performing the work;
(D) The map showing the location of the work area;
(E) The estimated cost of the alteration;
(F) Submission of evidence of town-designated levels of general liability insurance, Maryland Workers Compensation and any fees or bonds required by the town for the work;
(G) Any other information the town finds reasonably necessary to the determination of the permit being issued, including but not limited to licenses, traffic control, signage and safety measures, and evidence of equipment adequate to perform the job.
