549.33 PERMIT APPLICATION.
   (a)   The application for a temporary permit shall contain the following information:
      (1)   The name, social security number, date of birth, age, address, occupation and business address of the applicant if he is a natural person, or the name, address and principal place of business if the applicant is a firm or corporation.
      (2)   A notarized statement from the applicant verifying that the applicant, if a natural person, is not presently, and never has been, under medical care for any mental, alcohol or drug related disability.
      (3)   Written consent from the applicant for a criminal records check.
      (4)   A description of the dangerous ordnance for which a permit is requested.
      (5)   A description of the place or places where, and the manner in which, the dangerous ordnance is to be kept, carried or used.
      (6)   A statement of the purposes for which the dangerous ordnance is to be acquired, possessed, carried or used.
      (7)   Written statement from a geologist and a seismologist that no damage shall be sustained by any structure or building.
      (8)   Such other information as the Council may require in giving effect to this section.
 
   (b)   A temporary permit shall be issued if all of the following conditions are met:
      (1)   Investigation of the applicant by the Chief of Police.
      (2)   The applicant, if a natural person, is age eighteen (18) or over and if the application is for explosives only.
      (3)   It appears that the applicant has sufficient competence to safely acquire, possess, carry or use the dangerous ordnance and that the proper precautions will be taken to protect the security of the dangerous ordnance and insure the safety of persons and property.
      (4)   It appears that the dangerous ordnance shall be lawfully acquired, possessed, carried and used by the applicant for a legitimate purpose.
      (5)   Such other terms and conditions as may be required by Council.
      (6)   The application receives the intermediate approval of the Police Chief and City Manager. Final approval shall be by the Planning Commission only.
