Any person desiring a permit to install or replace any culvert shall file an application therefor with the Director of Public Works upon a form to be provided for that purpose. The application and the permit issued pursuant thereto shall be on the same form, which shall be substantially as outlined in Appendix B in the ordinance codified herein.
