No license shall be issued hereunder unless the applicant shall file with the application a certificate by an insurance company authorized to do business in the state, certifying that the applicant has the following minimum coverages:
(A) Bodily injury liability: $45,000 each occurrence;
(B) Property damage: $45,000 each occurrence;
(C) Means of support or loss of society: $55,000 each occurrence; and
(D) Combined single limit: in lieu of individual insurance coverage listed in divisions (A), (B) and (C) above, the applicant may provide a combined single limit policy in the amount of $300,000.
