Any person desiring a permit for any of the purposes mentioned in section 8-3-1 of this chapter shall make application therefor to the superintendent of streets, upon a form to be provided by said superintendent for such purposes. Such application shall state therein the purpose for which such excavation is to be made and the date and time it is proposed to begin such work, and shall be accompanied by a fee of one dollar ($1.00) for each such permit, where excavation is to be made in dirt street, and five dollars ($5.00) for each such permit when excavation is to be made in paved street.
