1341.04  APPLICATIONS.
   (a)   No applicant shall be given residential license unless he has furnished the following:
      (1)   The names of the applicant’s present and former employers and the period(s) of employment by each showing a minimum of three years experience in the trade for which he is applying for licensing.
      (2)   At least two letters of recommendation from previous employers who are licensed in the City of Campbell, and a letter of recommendation from an inspector currently licensed by the State of Ohio in the trade for which he is applying for license, or a letter of recommendation from a professional engineer licensed in the State of Ohio.
      (3)   Passed a test furnished by the City of Campbell Building Inspection Department for the trade for which he is applying, or is the holder of a current State of Ohio license for the trade for which is applying.
      (4)   Insurance and bond as listed in Section 1341.05 .
      (5)   A current Federal Tax Identification number.
      (6)   A current State of Ohio Worker’s Compensation Identification number.
      (7)   Fees as outlined in Section 1341.06 .
      (8)   All other pertinent information that the board may require.
         A.   The application and all required attachments must be reviewed and approved by the Chief Building Official or his approved agent, prior to the applicant being permitted to obtain a license.
         B.   A license may not be issued if it is proved to the satisfaction of the Chief Building Official that the applicant is or has been guilty of practices for which a registration may be suspended or revoked as listed in Section 1341.08 .
         C.   Renewal of Residential licenses must be accompanied by proof of 10 hours/year of continuing education from an approved training agency.
   (b)   No applicant shall be given commercial registration unless he has furnished the following:
      (1)   Insurance as listed in Section 1341.05.
      (2)   A current Federal Tax Identification Number.
      (3)   Proof of registration with the City of Campbell Tax Department.
      (4)   Proof of compliance with Worker’s Compensation.
      (5)   Proof of bond as outlined in Section 1341.05.
      (6)   Fees as outlined in Section 1341.06.
      (7)   All other pertinent information that the board may require.
