Application for a mechanical musical instrument license shall be made to the Mayor, upon such blank forms as shall be prescribed therefor. The applicant shall state his or its name; whether an individual, partnership or corporation, and if a partnership, the names and addresses of the partners composing the same; the address at which it is desired to maintain and operate the mechanical musical instrument; the nature of the business conducted at such place; the make, name, model, serial number, and other identifying information with reference to the particular mechanical musical instrument which it is desired to operate; the name and address of the owner of such instrument and such other and further information as the Mayor, in his discretion, may require.
