§ 112.28   LICENSE APPLICATION AND FEE.
   (A)   Any person, firm, or corporation desiring to engage in the business of installing and connecting waste sewers of existing residences and commercial buildings to public sanitary sewers shall, before so engaging in that business in the city, apply to the City Clerk for a license to engage in that business, and shall pay to the City Clerk at the time of making the application, a license fee in a sum as set by the City Council, and amended by resolution from time to time.
   (B)   The application for a license hereunder shall contain:
      (1)   The name and address of the applicant and all persons who have any interest, direct or indirect, in the business.  In the case of a corporation, all officers, directors, and stockholders shall be set forth;
      (2)   The address or location of the place of business;
      (3)   The record of any criminal convictions of all persons interested, directly or indirectly, in the business;
      (4)   Proof of good moral character of the applicant and all persons interested, directly or indirectly, in the business; and
      (5)   The application shall be signed by all persons whose identify must be set forth on the application, and the signature of all persons must be notarized by a notary public.
