§ 112.04   LICENSE APPLICATION; FEE.
   (A)   Any person, firm, or corporation desiring to engage in the business of installing and connecting watermain service leads to residences, commercial, and industrial buildings to the public municipal watermain shall, before so engaging in that business in the city, apply to the City Clerk for a license. All such applications shall be accompanied by a license fee which shall initially be set by the City Council, but which may be amended from time to time by resolution of the City Council.
   (B)   The application for a license hereunder shall contain:
      (1)   The name and address of the applicant and all persons who have any interest, direct or indirect, in the business.  In the case of a corporation, all officers, directors, and stockholders shall be set forth;
      (2)   The address or location of the place of business;
      (3)   The record of any criminal convictions of all persons interested, directly or indirectly, in the business; and
      (4)   The application shall be signed by all persons whose identity must be set forth on the application, and the signature of all persons must be notarized by a notary public.
