Any person desiring to operate an ambulance shall make an application therefor in writing to the Public Service-Safety Director. The application shall contain:
(a) The names and addresses of the person or persons interested in such application, including all partners if a partnership, and all officers and stockholders if a corporation, together with a full disclosure of their proportionate shares in the firm.
(b) The number of vehicles and equipment available as ambulances according to standards of the American Medical Association and the National Ambulance Association.
(c) A schedule of rates to be charged customers.
(d) The number of licensed drivers available.
(e) A statement indicating liability insurance of not less than one hundred thousand dollars ($100,000) for bodily injuries to or death of one person, three hundred thousand dollars ($300,000) on account of any one accident resulting in injuries to and/or death to more than one person and twenty-five thousand dollars ($25,000) on account of damages to property in any one accident.
(f) Communications facilities of the applicant.
(g) The proposed hours of operation.
(h) Amount of payment required of the City and bond of security to the City to secure such payment in the event of failure to operate an ambulance service during the franchise period. (Ord. 20-67. Passed 3-28-67.)
