§ 1028.06 QUALIFICATIONS FOR PROGRAM.
   (A)   In order to qualify for the senior citizen classification, the resident must do all of the following:
      (1)   Complete an application for city snow removal. Said application shall contain the following certifications:
         (a)   That the resident and his or her spouse are retired and live alone;
         (b)   That there is no able bodied person residing with the resident who is capable of removing snow;
         (c)   Proof of age (birth certificate, driver’s license and the like); and
         (d)   That the total household income qualifies under the city’s eligibility requirements, with documentation, per the guidelines referred to in § 1028.08. Total income includes Social Security.
      (2)   Execute a consent and release form, provided with the application, granting permission to the city to provide such service and exonerating the city and the service provider from any and all liability for providing such service.
   (B)   In order to qualify for the physically disabled citizen classification, the resident must do all of the following:
      (1)   Complete an application for city snow removal. Said application shall contain the following certifications:
         (a)   That the resident and his or her spouse are living alone;
         (b)   That there is no able bodied person residing with the resident who is capable of removing snow;
         (c)   That the total household income qualifies under the city’s eligibility requirements, with documentation, per the guidelines referred to in § 1028.08. Total income includes Social Security; and
         (d)   A doctor’s letter stating the exact type of disability.
      (2)   Execute a consent and release, provided with the application, granting permission to the city to provide such service and exonerating the city and the service provider from any and all liability for providing such service.
