On completion of the work or repairs by the applicant which necessitated the street cut or excavation, the applicant shall make reclamation of the street in the area of the cut or excavation within 45 days therefrom according to the following specifications:
(A) Backfill with a minimum of 12 inches of sand above any underground water, sewer or public utility lines;
(B) Apply or fill the cut or excavation with #53 stone;
(C) When the above material has settled or subsided sufficiently to avoid further subsidence, apply a minimum of 4 inches of finish blacktop to the affected area;
(D) Before the applicant may be released from further obligation of reclamation, the Street Department Supervisor must approve the reclamation performed; and
(E) Failure of an applicant to properly make proper reclamation may result in the denial of issuance of any future street cut permits to the applicant.
