§ 155.03  APPLICATION; FEES.
   (A)   Any person desiring to make a cut or excavation into a public street shall, before making the cut or excavation, apply for a street cut permit with the City Clerk-Treasurer on the prescribed form available at the office of the Clerk-Treasurer by completing the form and providing the information with the Clerk-Treasurer along with the permit application requested therein; filing the application fee of $25, a plat map or other scaled drawing indicating the exact location of the proposed cut or excavation, and the proper bond.
   (B)   The street cut application form shall contain the following information:
      (1)   Name, address and telephone number of the person applying for the permit;
      (2)   The specific reason or reasons for which a street cut permit is being requested;
      (3)   The size of the cut or excavation, including length, width and depth;
      (4)   Name, address and telephone number of the person or company contracted by the applicant who will perform the actual cut or excavation if it is to be done by someone other than the applicant;
      (5)   The specific date that the cut or excavation is to be made or recommenced and the late (or anticipated date) of completion of the work necessitating the cut;
      (6)   The name, address and telephone number of the person or company contracted by the applicant who will be performing the street reclamation following completion of the work; and
      (7)   A statement that the applicant agrees to hold harmless and indemnify the city against any and all damages and claim for damages which may be asserted against the city or its officers, employees or agents by reason of or arising out of the street cut or excavation and any work done as a result thereof.
   (C)   On the proper submission of the street cut application, the application fee, plat map or scaled drawing and bond to the City Clerk-Treasurer at least 10 days prior to the scheduled cut or excavation, the Clerk-Treasurer shall issue a street cut permit to the applicant within at least 7 days prior to the commencement of the cut or excavation after obtaining approval of the Street Department Supervisor.
   (D)   A copy of the permit shall be supplied to the Street Department Supervisor, the Police Chief, the Fire Chief and the Mayor by the Clerk-Treasurer.
