§ 114.08 APPLICATION FOR DRIVER’S LICENSE.
   (A)   An application for a taxicab or public transit driver’s license shall be filed with the Taxicab License Board on forms provided by the city. The application shall be verified under oath, and shall contain the following information: the name, home address, all places of residence and all employment during the preceding five years and a personal description of the applicant; the experience of the applicant in the transportation of passengers; the educational background of the applicant; a concise history of his or her employment, and the names and addresses of four persons who have known the applicant for a period of five years, and who will vouch for the sobriety, honesty, and good character of the applicant.
   (B)   Each applicant shall furnish a recent photograph, approximately three inches square, and fingerprints in duplicate. At the time the application is filed, the applicant shall have in his or her possession and show to the Board an Ohio driver license which is not revoked, and shall pay to the Board the fee as set forth in § 35.70. Before approval is given by the Taxicab Review Board, the applicant shall provide a certificate from a physician licensed by the State of Ohio Medical Board certifying that in his or her opinion the applicant is not afflicted with any disease or infirmity which might make the applicant an unsafe or unsatisfactory driver.
