§ 114.03 APPLICATION FOR OWNER’S OR PUBLIC TRANSIT PROGRAM OPERATOR’S LICENSE.
   (A)   An application for a taxicab owner’s license shall be filed with the Taxicab License Board by one or all owners of the taxicab company upon forms provided by the city.
   (B)   An application for a public transit program operator’s license shall be filed with the Taxicab License Board by one or all owners of the transportation service provider with whom the city has contracted for public transit service.
   (C)   The application shall be verified under oath and shall furnish the following information: the name and address of the applicant; the number of vehicles to be operated or controlled by the applicant and the location of the proposed depots and terminals; the length of time that the applicant has been a bona fide resident of the city, and other places where the applicant has resided during the past ten years; the color scheme or insignia to be used to designate the vehicle or vehicles of the applicant; all information which is hereafter required for a driver’s license, and further information as the Taxicab License Board may require.
