§ 50.400 TERMINATION OF WASTEWATER PERMIT OR PERMISSION TO DISCHARGE.
   (A)   The POTW Director may revoke a watewater permit or permission to discharge for good cause, including but not limited to, the following reasons:
      (1)   Failure to accurately report the wastewater constituents and characteristics of the individual wastewater contributor’s discharge;
      (2)   Failure to report significant changes in operations, or in wastewater constituents and characteristics;
      (3)   Refusal of reasonable access to the contributor’s premises for the purpose of inspection or monitoring; or
      (4)   Violations of conditions of the wastewater permit or permission to discharge, conditions of this chapter, or any applicable state and federal regulations; or
      (5)   Failure to submit the written statement required by § 50.399.
   (B)   A noncompliant industrial contributor will be notified of the proposed termination of its wastewater permit, and will be offered an opportunity to show cause under §§ 50.395 through 50.400 of this chapter why the proposed action should not be taken.
