(A) Any person desiring a license for an automatic food vending machine or machines shall make application in writing to the City Clerk.
(B) The application shall set forth the number of automatic food vending machines intended to be installed, the location at which each is to be installed, and a description of each.
(C) The description shall contain the manufacturer’s name, the make and factory number, whether a single or multiple machine, and the monetary value of the coin or coins used to operate the machine.
(D) The application shall also set forth further information required by the Health Commissioner concerning the type or types of machines and the kind or kinds of food to be dispensed therefrom.
(E) Before any license is issued, the Health Commissioner shall cause an inspection to be made to determine the character and fitness of the machine or machines from the standpoint of insuring protection of the food from improper handling, dust, dirt, flies, vermin or other contamination.
