No person shall conduct or operate the business of food dispenser without first having obtained a license. Written application, which shall conform to the general requirements of this subchapter relating to applications for licenses, shall be made to the City Clerk. The application shall contain the name and residence of the applicant and, if the applicant is a firm, of each member of the firm. If the applicant is a corporation, the application shall contain the names and residences of its principal officers. The application shall contain such further information as may be required by the Commissioner of Health in order to inform the Commissioner fully of the size and nature of the place to be used for the purposes of the business and the conditions, equipment and facilities for conducting the business therein, the health of persons to be employed and the regulations and facilities provided for them.
