4-2-6: APPLICATION FOR LICENSE:
   A.   Contents: Written application for a retail liquor dealer's license shall be addressed to the local liquor control commissioner and filed with the village clerk, signed by the applicant if an individual, or by all partners if the applicant shall be a corporation, verified by oath or affidavit, and shall contain the statements and information as set forth in this subsection. For the purposes of the requirements set forth in this section, the term "applicant", as used in this subsection, shall include any and all persons, including corporations, required to be identified in subsection A1 of this section.
      1.   For all applicants (including partners, stockholders, managers, agents and bartenders as set forth below), the full name, including middle initial, date of birth, home and work addresses and home and work phone numbers, and social security numbers; in the case of a partnership, the persons entitled to share in the profits thereof; in the case of a corporation, the date of incorporation, the state of incorporation, and if such state be other than Illinois, upon which the corporation was certified as a foreign corporation entitled to conduct business in the state of Illinois, the names and addresses of the officers and directors, the names and addresses of all stockholders if the total number of stockholders is twenty (20) or less, but if the total number of stockholders is more than twenty (20), only those stockholders owning more than a five percent (5%) interest in the corporation shall be required to be listed; in all cases, the names of and information required in this section and section 4-2-5 of this chapter for any and all managers or agents conducting the business and bartenders.
      2.   Citizenship of applicant and place of birth; and if a naturalized citizen, the time and place of naturalization.
      3.   Character of business of the applicant; and in the case of a corporation, the objects for which it was formed.
      4.   Length of time the applicant has been in business of that character; or in the case of a corporation, the date when its charter was issued, and a copy of the articles of incorporation.
      5.   Amount and nature of goods, wares and merchandise on hand at the time application is made.
      6.   Location and description of the premises or place of business which is to be operated under such license.
      7.   Statement whether or not any applicant has made application for a similar or other license on premises other than that described in the application and the disposition of such other application.
      8.   Statement whether or not any applicant has ever been convicted of a felony or is otherwise disqualified to receive a license by reason of any matter or thing contained in this chapter or any other ordinance of the village, or contained in the laws of the state of Illinois.
      9.   Statement whether or not any applicant's previous license issued by any state or subdivision thereof or by the federal government has been revoked and the reasons therefor.
      10.   Statement that the applicant will not violate any of the laws of the state of Illinois, or of the United States, or any ordinance of the village in the conduct of his place of business.
      11.   Evidence that the applicant has in force the insurance required by statute and this chapter. (Ord. 2003-40, 10-27-2003)
   B.   Form; Review; Compliance: Applications for a retail liquor dealer's license shall be on a form consistent with the terms of this chapter and available from the village clerk. A copy of every completed application for a retail liquor dealer's license shall be forwarded by the village clerk, upon receipt, to the local liquor control commissioner and to the village administrator and the police department for their review, the results of which shall be promptly forwarded to the local liquor control commissioner. No license shall be issued of any class by the local liquor control commissioner unless the application complies with all requirements of this chapter and the laws of the state of Illinois.
   C.   Service Fee; Legal, Planning And Consultant Fees: In the event that an applicant seeks a new liquor license classification or an amendment to an existing classification, the applicant shall pay a five hundred dollar ($500.00) service fee. The applicant is further responsible for any consultant review fees incurred by the village in excess of the initial service fee. No consideration shall be given to a new application for a new liquor license classification or amendment to an existing classification until such service fee is paid, and no license shall be granted to the applicant upon approval of the classification or amendment until such time as the applicant has paid all legal, planning and consulting fees.
