6-4-4: PUBLIC DISPLAYS OR DISCHARGE OF DANGEROUS, AGRICULTURAL OR WILDLIFE FIREWORKS:
   A.   Application For Permit; Fee: Any adult person or other group desiring to make a public display of fireworks, or use or discharge dangerous, agricultural or wildlife fireworks, shall first make written application to the city clerk for the issuance of such a permit allowing such person to purchase such fireworks, and to use and discharge the same, such application to be accompanied by a filing fee as specified in section 3-4-1 of this code. In the event no such permit is issued, the filing fee shall be returned to the applicant. Such application shall show the following information:
      1.   Name and address of applicant;
      2.   If an organization, the purpose for which applicant is primarily existing and for which it was organized;
      3.   The names and addresses of the officers, if any, of the applicant;
      4.   The location where applicant will display said fireworks or use the same, together with the quantity and description of fireworks applicant desires to purchase, use and discharge, and the location, description and quantities of any fireworks stored off site;
      5.   When and where the applicant was organized and established, or if a natural person, the applicant's age which must be at least twenty one (21) years;
      6.   The location of the applicant's principal and permanent place or places of business;
      7.   The name and address of any wholesaler or distributor from whom the applicant intends to purchase such fireworks;
      8.   The manner, method and times when the applicant proposes to use such fireworks;
      9.   Such other information as the chief of the fire department may require in order to make his investigation.
   B.   Term Of Permit: Such permit, if so issued, shall be valid for only the period of time for which issued. (2003 Code § 3-03-02)
