4-10-5: CONTENTS OF APPLICATION:
Application for a special event permit shall be made on forms provided by the city clerk. The application shall be signed by the applicant and shall contain the following:
   A.   The name, address, and telephone number of the applicant, and if the same be a corporation, the names of its principal officers, or if the same be a partnership, limited liability company, association, or organization, the name or names of the person or persons who shall be responsible for the event. This information may be used by the city clerk to conduct a criminal background check.
   B.   A description of the special event proposed to be held, estimates of: 1) the number of persons participating in the event; 2) the number of spectators and vehicles anticipated; 3) the number and type of participating vendors anticipated, the purpose of the event, the date, hours, and location where the event is proposed to be held, and the admission fee, if any, to be charged.
   C.   Verification of conformance to department of health and/or adopted building codes in regard to sanitary facilities.
   D.   A description of the equipment to be used, if any, including the number and types of vehicles, the number and types of sound amplifying systems, whether mounted, portable, or stationary, the time and area of operation of the sound amplifying systems, the number and type of special units such as musical groups, animal petting zoos, or shows and the like.
   E.   An agreement that the city shall be compensated for any damage to public property, and that the public property shall be cleaned and restored to the condition in which it was found prior to the holding of the special event.
   F.   "Security plan", as defined in section 4-10-2 of this chapter.
   G.   Verification of conformance to fire codes.
   H.   A detailed drawing or schematic of the staging area and the entire event grounds, showing emergency lanes for responding vehicles, the exits for the public, emergency lighting locations (if required), fire extinguisher locations, and emergency medical service locations (if required).
