The driver of a vehicle involved in an accident resulting in injury to or death of any person or property damage to an apparent extent of $100 or more, shall:
(A) Immediately give notice of the accident to the Police Department; and
(B) (1) Within five days after the accident, forward a written report of the accident to the Police Department or a copy of the report required by the state.
(2) This report shall not be required if the accident is investigated at the scene by a police officer while the driver is present.
