§ 53.021 DETERMINATION OF SIGNIFICANT INDUSTRIAL USERS.
   (A)   All users or prospective users which generate non-domestic wastewater shall, when making application for discharge, pursuant to § 53.020, provide the following information:
      (1)   Name of owners and operators;
      (2)   Address;
      (3)   Location, if different from the mailing address, the Standard Industrial Classification SIC of the operation(s) carried out by the applicant;
      (4)   Wastewater volume;
      (5)   Wastewater constituents and characteristics;
      (6)   Discharge variations;
      (7)   A description of all toxic materials handled on the premises; and
      (8)   Any other information deemed necessary by the plant manager, including, but not limited to site plans, floor plans, mechanical and plumbing plans and details of existing and proposed pretreatment and/or equalization facilities.
   (B)   If the plant manager determines that the applicant constitutes a significant industrial user, then the conditions of § 53.022 shall apply.
