(A) When public improvements are to be constructed pursuant to a contract with the city and public bids for the construction have been solicited and are to be received by the city, the bids shall include the following:
(1) Bid security as may be prescribed by the city's specifications for the work to be done;
(2) A bidder's statement of non-collusion as required in the city's specifications for the work to be done;
(3) A bidder's business relationship affidavit as required in the city's specifications for the work to be done; and
(4) A non-discrimination affidavit as required in the city's specifications for the work to be done.
(B) Any failure to provide the information required in this section shall be sufficient reason for rejecting a bid.
