§ 847.03 LICENSE APPLICATIONS, INVESTIGATIONS AND ISSUANCE.
   (A)   A person who wishes the license required in § 847.02 shall apply in writing to the City Collector and shall truly state in his or her application the location or proposed location of the hospital, the purpose for which it is to be opened, conducted or maintained, the accommodations or proposed accommodations for the inmates thereof, the nature and kind of treatment given or proposed to be given therein and the names and addresses of the chief physician or surgeon, intended chief physician or surgeon or board of physicians or surgeons attendant therein.
   (B)   The Mayor, upon the presentation of the application, shall make or cause to be made a strict inquiry into the facts set out in the application. The Mayor shall issue or cause to be issued the desired license if the applicant pays the license fee provided for in § 847.04 and if, upon the inquiry, he or she finds that:
      (1)   The hospital is or is intended to be so conducted as to afford proper accommodations for the care of the persons received or proposed to be received therein;
      (2)   The chief physician or surgeon, intended chief physician or surgeon or board of physicians or surgeons thereof gives or is under agreement to give the attendance therein as does or will render him or her or them responsible, professionally, for the medical or surgical treatment given or to be given to any and all patients therein;
      (3)   The chief physician or surgeon, intended chief physician or surgeon or board of physicians or surgeons is regularly authorized to act as such under the laws of the state; and
      (4)   The hospital complies or is intended to comply with all the rules and regulations then in force concerning the management and control of a hospital.
   (C)   No license shall be issued or renewed under this section until the hospital has been inspected and approved by the Board of Health.
