Any individual or legal entity desiring a license as required in § 120.15 shall file an application therefor with the Village Comptroller. Such application for a taxicab license shall be in writing and shall include the following information:
(A) The full name, address and date of birth of the applicant;
(B) The year, make, condition and type of vehicle used;
(C) The factory number thereof;
(D) The state license number thereof;
(E) The seating capacity thereof, according to the trade rating;
(F) The length of time the vehicle has been in use and mileage on the vehicle;
(G) Color scheme of cab used by the operator and trade name;
(H) The experience of the applicant and drivers in the transportation of passengers;
(I) The number of vehicles to be operated or controlled by the applicant and the location of the proposed terminal;
(J) A certified financial statement of the applicant, including the amount of unpaid judgments against the applicant and the nature of the transaction or acts giving rise to such judgments;
(K) If applicant is not an individual, the names and addresses of its principal officers;
(L) A list of all proposed drivers of taxicabs;
(M) A certification from the Chief of Police certifying that all taxicabs covered under the license have been inspected and comply with all requirements of this chapter;
(N) Such further information as the Village Comptroller may reasonably require.
