(A) A physician or other person with knowledge of any person sick with any disease posing an immediate danger to the public health shall forthwith report the same to the Director. The Director shall at once notify the State Department of Health of the existence of such disease, and shall subsequently cause a full report to be made to the Secretary of the State Department of Health upon the blank forms furnished for that purpose.
(B) Every practicing physician in the city who shall have a patient therein or within three miles thereof affected with any disease posing an immediate danger to the public health shall forthwith make report thereof in writing to the Director, giving the name and address of such patient.
