(A) The driver of a vehicle which has collided with, or been in an accident with any vehicle, person or property in such a manner as to cause injury to or damage shall stop immediately and render such assistance as may be possible, and give his or her true name and residence to the injured person or any other person requesting the same on behalf of the injured person, or the owner of the property damaged, and to a police officer, if one is present.
(B) The driver of each vehicle concerned in accident involving bodily injury or death or damage to the property of any one person in excess of $100 shall report such accident to the Police Department in writing on forms provided for such purpose as soon after the accident as may be practicable, but in no event less than 24 hours after such accident.
