At any hearing of the Board of Appeals, the following procedures shall be followed:
(a) Testimony of the appellant, the City and any witnesses shall be recorded.
(b) The appellant, or the authorized agent of the appellant, and the City employee who issued the order or notice shall be present.
(c) A quorum of the Board shall be present.
(d) Minutes shall be prepared which identify all parties present, accurately summarize all pertinent statements made, include all evidence and records submitted, show all motions and actions of each member and record the vote of each member.
