If the Administrator declares a dwelling, dwelling unit, or structure unsafe or unfit for occupancy and is unable to personally serve the responsible party with a notice of the unsafe or unfit conditions, the Administrator shall send the responsible party the notice by certified mail and shall post a notice in a conspicuous place on or near the affected premises. The notice shall state the following:
(a) The address of the dwelling, dwelling unit, or structure;
(b) The name of the responsible party;
(c) The date by which the violations are to be corrected; and
(d) How further information may be obtained.
