§ 152.070  RECORD PLAT FORM; INFORMATION TO BE CONTAINED THEREON.
   (A)   Form.  The record plat shall be legibly drawn on Mylar or other reproducible plastic film of equal permanence. The size of the plat shall be 18 by 28 inches. The plat of a subdivision containing six acres or less shall be drawn at a scale of one inch equals 50 feet. All other subdivisions shall be drawn at a scale of either one inch equals 50 feet or at a scale of one inch equals 100 feet except that subdivisions involving “deed out” parcels or other information which would not be accurately represented at the aforementioned scales may be drawn at such a scale as to accurately represent such parcels or information. If more than one sheet is needed, each sheet shall be numbered and the relation of one sheet to another clearly shown.
   (B)   Information to be contained on record plat.  The final record plat shall contain the minimum information required by R.C. §§ 711.01 through 711.04 inclusive, and shall also contain the following information:
      (1)   Name of the subdivision, location by military survey and township, date, north point and scale;
      (2)   Total acreage of entire tract, total acreage of land contained in streets, total acreage of land contained in lots, total acreage of open space, park land and the like;
      (3)   Names and addresses of the subdividers, and the registered surveyor who prepared the plat and appropriate registration numbers and seals;
      (4)   Plat boundaries, based on accurate traverse, with directional and lineal dimensions. All dimensions, both directional and linear shall be determined by an accurate control survey in the field which must balance and close within the limit of one in 5,000, one in 10,000 adjusted;
      (5)   Bearings and distances to nearest established street lines, military survey or township lines or other recognized permanent monuments which shall be accurately described on the plat;
      (6)   Municipal, military survey or township lines accurately tied to the lines of the subdivision by distances and bearings;
      (7)   Exact locations, rights-of-way and names of all streets within and adjoining the plat;
      (8)   Radii, deflection angles, points of curvature, tangent bearings, lengths of arcs and lengths and bearings of chords of all applicable streets within the plat area;
      (9)   All easements for rights-of-way provided for public services or utilities, and any limitations of such easements;
      (10)   All lot numbers and lines with accurate dimensions in feet and hundredths, with bearing or angles to street lines. When lots are located on a curve, the lot width at the building setback line shall be shown. Area of all individual lots in acres;
      (11)   Building setback lines with dimensions;
      (12)   Accurate location and description of permanent cornerstones and concrete markers which shall be located and set in accordance with the minimum requirements of R.C. § 711.03 and shall in all instances be located and set to establish all boundary lines of the subdivision, all intersections and all centers of cul-de-sacs;
      (13)   Accurate location and description of iron pins which shall be required to be located at all lot corners;
      (14)   Accurate outlines of areas to be dedicated or reserved for public use, or any area to be reserved for the common use of all property owners. The use and accurate boundary locations shall be shown for each parcel of land to be dedicated;
      (15)   Location and dimensions of common parking areas, alleys, pedestrian walks and points of circulation ingress and egress;
      (16)   Should the record plat involve a planned unit development or a special development, those items which may be required to be shown on the record plat by the appropriate zoning or platting authority;
      (17)   Location and/or statement of adequate outlet for storm sewer as approved by the Village Engineer;
      (18)   Restrictions and covenants the subdivider intends to include in the deeds to the lots of the subdivision;
      (19)   A statement certifying the subdivision satisfies all applicable requirements of the zoning district governing the use of the land at the time of recording;
      (20)   Certification by a registered surveyor to the effect that the plat represents a survey made by the surveyor and that the monuments shown thereon exist as located and that all dimensional details are correct;
      (21)   A notarized acknowledgment of all owner(s) and lien holder(s) to the plat and its restrictions including dedication to public use of streets, alleys, parks or other open spaces shown thereon and the granting of required easements; and
      (22)   Approval and acceptance clauses for the signatures of a representative of the Planning Commission, the Village Engineer and Council. The signature of the Zoning Inspector shall be required on record plats of planned unit developments.
