§ 152.066  INFORMATION REQUIRED.
   (A)   Information to be included on final plan is as follows:
      (1)   Name of the subdivision, section number of the subdivision if final plan constitutes only one portion of a subdivision;
      (2)   Location by military survey or other survey;
      (3)   Names and addresses of the subdividers, and the professional engineer and/or registered surveyor who prepared the plan and appropriate registration numbers and seals;
      (4)   Date of survey;
      (5)   Scale of plan and north point;
      (6)   Boundaries of the section for which approval is sought shown in relationship to the entire subdivision;
      (7)   Exact locations, right-of-way and names of all streets within the subdivision;
      (8)   Final lot layout, final lot numbers, approximate area in square feet of each lot;
      (9)   Building setback lines; and
      (10)   Accurate outlines of areas to be dedicated or reserved for public use or any area to be reserved for the common use of all property owners.
   (B)   It should be noted the information on the final plan shall be a representation of the subdivision as the improvement drawings call for it to be constructed. The final plan shall show all modifications required by the Planning Commission in its approval of the preliminary plan. A resubmitted preliminary plan will not be accepted in lieu of a final plan.
