§ 152.053  SUPPLEMENTARY INFORMATION.
   The following information shall be supplied in addition to the requirements in § 152.052.
   (A)   One fully completed copy of the Village Planning Commission preliminary plan application form;
   (B)   Five copies of a preliminary drainage plan illustrating the manner in which the applicant intends to accommodate surface drainage through and within the proposed subdivision and if necessary, what measures will be undertaken to control erosionary forces. The Planning Commission may in some instances require temporary seeding in those areas disturbed during the construction period. Drainage from one watershed shall not be diverted to another watershed;
   (C)   A copy of proposed covenants and restrictions;
   (D)   A comprehensive identification and analysis of the operating characteristics of a proposed land use, building or structure. Such statements specifically include the characteristics and method by which the proposal will account for:
      (1)   Fire hazards;
      (2)   Radioactivity or electrical disturbance;
      (3)   Noise;
      (4)   Vibration;
      (5)   Air pollution;
      (6)   Glare;
      (7)   Water pollution; and
      (8)   Related or pertinent operational characteristics and performances.
   (E)   In a letter accompanying the request for approval of the preliminary plan, the subdivider shall state the type of sewage disposal he or she proposes to use. If other than public sewers or a treatment plant, it shall be accompanied by a letter from the County Health Department stating what type of sewage disposal will be approved for the soil and drainage conditions encountered in the proposed subdivision. A central sewage treatment plant and a central water system shall be constructed by the subdivider, when required to meet appropriate Board of Health Regulations. If a central treatment plant and/or central water system is to be used, a proposal shall be included discussing the method and cost for the incorporation of such system into the village water and sewer system;
   (F)   Should the preliminary plan involve a portion of a planned unit development or a special development, those items which are required for review by the appropriate zoning or platting authority shall be required to be submitted to the Planning Commission; and
   (G)   Any additional information that may be deemed necessary for the Planning Commission to make a reasonable decision when a proposed development presents difficult or unusual problems.
