§ 152.052  INFORMATION REQUIRED.
   Information to be contained on the preliminary plan is as follows:
   (A)   Proposed name of the subdivision, which shall not duplicate or too closely approximate, phonetically or in spelling, the name of any other subdivision in the county;
   (B)   Location by military survey or other survey;
   (C)   Names, addresses and phone numbers of the owner, subdivider, professional engineer and registered surveyor who prepared the plan, and appropriate registration numbers and seals;
   (D)   Date of survey;
   (E)   Scale of plan and north point;
   (F)   Boundaries of the subdivision and its acreage;
   (G)   Names of adjacent subdivisions, owners of adjoining parcels of unsubdivided land and the location of their boundary lines;
   (H)   Locations, widths and names of existing streets, railroad rights-of-way, easements, parks, permanent buildings and corporation, township and county lines; location of wooded areas and other significant topographic and natural features within and adjacent to the plan for a minimum distance of 200 feet;
   (I)   Zoning classification of the tract and adjoining properties and a description of proposed zoning changes if any;
   (J)   Existing contours at two-foot intervals for predominant ground slopes within the subdivision between level and 10% grade and not greater than five foot intervals for predominate ground slopes within the subdivision over 10% grade;
   (K)   Existing sewers, water lines, culverts and other underground structures, and power transmission poles and lines, within and adjacent to the tract;
   (L)   Location, names and widths of proposed streets and easements;
   (M)   Building setback lines with dimensions;
   (N)   Location and dimensions of all existing and proposed utility and sewer lines, showing their connections with existing systems;
   (O)   Layout, numbers and approximate dimensions and sizes of each lot;
   (P)   Parcels of land in acres to be reserved for public use or to be reserved by covenant for residents of the subdivision;
   (Q)   Proposed use of lots, type and number of dwelling units, type of business and industry;
   (R)   Location and approximate dimensions of all existing buildings;
   (S)   Location, dimension and approximate grade of proposed parking and loading areas, alleys, pedestrian walks, streets and other points of circulation ingress and egress to the development; and
   (T)   Vicinity map illustrating the proposed subdivision’s relation to surrounding environmental, land use, circulation, infrastructure and property characteristics. Vicinity maps shall be legible and may be of composite overlays of soil, NGS and property data maintained by the Planning Commission. The scale shall be sufficient to illustrate the environs of the proposed subdivision, but shall not be of smaller scale than one inch equals 2,000 feet.
