3-3-3: PERMIT APPLICATION:
   (A)   Application for a solicitor's permit shall be made in writing, notarized, on a form provided by and filed with the village police department. Each applicant shall state the following information under oath:
      1.   The applicant's full name, all addresses of applicant's residence during the past two (2) years, the length of time of the applicant's residence at each address, and the applicant's business address if other than their current residence address;
      2.   The applicant's place and date of birth, social security number, driver's license number and state of issuance or current state or federal identification card, and physical description of applicant;
      3.   The name and address of the business or other person or entity by whom the applicant is employed or whom the applicant represents, and the initial date of such employment or representation;
      4.   A description of the nature of the applicant's solicitation;
      5.   The dates for which the applicant seeks a solicitor's permit;
      6.   Information, including date information, regarding any prior application or use of a solicitor's permit by applicant in the village, or any revocation of a solicitor's permit issued by the village to applicant;
      7.   Information regarding any prior conviction of the applicant of a felony under any state or federal law; and
      8.   Information regarding prior or current violations, if any, by the applicant of any of the provisions of this chapter, or of the solicitation regulations of any local, state, or federal government.
   (B)   Village police department personnel will photograph each applicant and affix such photograph to applicant's solicitor's permit, if issued.
   (C)   Each application shall be accompanied by a fee of twenty five dollars ($25.00) for the investigation and administration of the provisions of this chapter. (Ord. 03-18, 9-4-2003)
