153.03 GROUP HEALTH INSURANCE.
   (a)    The following contracts providing health benefits and coverage are hereby approved by Council:
      (1)   Health Plan of the Upper Ohio Valley.
      (2)    Agway.

   (b)    The Village shall pay premiums, for each person hired before February 1, 1995, listed below under subsection (c) hereof, up to two hundred twenty-five and 23/100 ($225.23) per month for family coverage and up to ninety and 08/100 ($90.08) per month for single coverage from the funds as designated below.

   (c)    Police, Street, Fire Chief, Park, Fiscal Officer, Cemetery Caretaker: General Fund.
      Water Department Employees: Water Fund.
      Sewer Department Employees: Sewer Fund.
      Income Tax Department Employees: Income Tax Fund.

   (d)    In the event the health insurance premiums for any person hired before February 1, 1995, exceed the amounts listed under subsection (b) hereof, the Village shall pay in addition to the amount listed under subsection (b) hereof, eighty percent (80%) of the additional premium due. The Village is hereby authorized to withhold from any employee’s wages an amount which is equal to twenty percent (20%) of the amount due after subtracting the amount payable by the Village under subsection (b) hereof.

   (e)    In the event the premium paid for any person's coverage by the Village as set forth in subsection (c) hereof, is less than the amounts herein set forth and during the term of the applicable contract that person, or his or her dependent, becomes entitled to benefits thereunder, the Village will pay any portion deductible from such benefits by virtue of the contract provisions but not in excess of the difference between the maximum amount coverage for that person which would have been payable by the Village during the contract term and the actual premium liability of the Village for that person during the contract term.

   (f)    In the event a village employee, who is in the Military Reserves or National Guard is called to active duty, that person's family health insurance premiums shall continue to be paid one hundred percent (100%) by the Village. Such insurance premiums shall continue for so long as the employee is on active duty. (Ord. 1840. Passed 4-15-91.)
   (g)   For any employee hired on or after February 1, 1995, the Village shall pay eighty percent (80%) of the health insurance premium (single or family coverage) out of the fund as designated in subsection (c) hereof. The employee shall be responsible for the other twenty percent (20%) of said premium, and the Village shall withhold these amounts from the employee’s wages. (Ord. 1979. Passed 3-13-79.)
   (h)   All Village employees reaching the age of sixty-five years shall be required to enroll in Secure Care or such other group health insurance plan for individuals over the age of sixty-five that may then be offered by the Village. Such enrollment shall take place within thirty days of the employee’s sixty-fifth birthday. Current employees shall enroll within thirty days of the effective date of this section.
