(A) Upon application for the license required by § 872.02, and at each annual review thereafter, the operator shall submit to the Health Department a license fee made payable to the city. This fee, or any portion thereof, shall not be refunded if the license is denied, terminated or revoked.
(B) The license fee may be waived when the petitioner is a local, state or federal government agency.
(C) The initial application fee shall be $100 for all facilities. The annual renewal fee shall be $50 for all facilities.
(D) Any new facility desiring to begin operation within three months of the annual renewal date may, at the discretion of the Health Commissioner, be granted a license for not to exceed a 15-month period. Such license fee shall be prorated for the period.
