935.02 PROOF OF INCOME.

   To demonstrate that his/her household qualifies for the exemptions and/or benefits set forth in this chapter, a member of each household shall, by May 1st of any year, submit an application as prescribed by the City, and such additional information, including a signed and verified affidavit, as the City deems necessary to demonstrate that his/her household's total annual income does not exceed the level set forth in Section 935.01.
